Santa Clara ESD Pre-Enrollment
Requirements to Register New Students
To complete your child's registration the parent or guardian must upload the following documents:
- Original Birth Certificate or Passport
- Immunization Record (Each student enrolling for the first time in a district or secondary school, preschool, or child care and development program or, enrolling or advancing to grade 7, shall present an immunization record from any authorized private or public health care provider, certifying that he/she has received all required immunization in accordance with law. Students shall be excluded from school or exempted from immunization requirements only as allowed by law.
- Residence Verification (Santa Clara Elementary School District may ONLY enroll students whose parent(s) or guardian(s) reside within school district boundaries (Education Code 48204) or accepted through District of Choice. Residency verification is a parent responsibility and falsification of information provided will be grounds for immediate cancellation of enrollment.
- Accepted Proof of Residency; Must be dated within 30 days and Must have the parent/guardians' name on the document. Gas, Water, Electricity, Rental / Lease Agreement, Mortgage Statement, Drivers' License or Government ID, Pay Stub, Government Agency Correspondence. Please note that all residence verifications are subject to physical verifications by district personnel.)
- Parent/Guardian Driver's License or other Picture ID (Guardianship documents are needed if you are the guardian of the student being enrolled. If you do not have guardianship paperwork and are the caregiver for the student, you must complete a Caregiver's Authorization Affidavit. The Caregivers Authorization is available in the school office or on the school's website and needs to be renewed each school year that the student attends SCESD.)
- Custody Documents (if needed - Please bring the original current court documents, copies will be made for your student's file. Custody issues can only be enforced by the school if the custody documents are on file with the school)
- Special Education (If your student has an IEP (Individualized Education Plan) or 504 Plan, please submit a copy with registration.)
